Once payment is received you will receive via e-mail (within 24 hours)

forms that will need to be filled out and returned:
1. A questionnaire for you

2. An optional doctor's release.**
We will then schedule a time for a telephone intake interview
(conversation), so that you can tell me exactly what you want, what your
challenges are and what your specific healing goals are.
I will
then write your script and record your CD program.
